We provide full support for individuals after being discharged from the Hospital through the entire process and home return.

Live-in Care

With our live-in care services, our staff will move into your home with you to support you to remain living independent. All care given is agreed in the individual Care Plan.

Night Care

Palliative Care

Palliative care service is provided in the comfort of your home in cases where patients are suffering from life threatening illness, whatever your requirements are.

Specialist Care

We have developed a comprehensive person-centered plan of care based on identified personal needs. Regular care plan review is carried out to ensure that it is suitable for every individual’s health and social care needs.

Personal Care

Our discreet personal care service is designed to support you with day- to-day-living support. This allows you to remain independent in the comfort of your own home.

Sitting/Respite Care

From 1 hour – 24 hours in the absence of the mail carer.

Social Companionship

Our service can meet the simplest requirements. We provide support be it outings, household chores, someone to have a chat with or cup of tea with.

Our Accommodation

We provide Accommodation that resembles a family home where they a nurtured and supported in a safe environment. Depending on your needs you may be placed in solo accommodation or living with other young people who will be suited to you.
There will also be a support worker on site to ensure that you are safe and secure. You will have your own room with your own key to your bedroom. We have shared or communal areas such as sitting rooms, dining room and kitchen providing you with the opportunity to interact with others.

Transportation

Transport can be provided, or a driver is preferred for wherever you would like to go.

Supported Living

At Elite Medical NW we provide Independent Supported Living Accommodation to young people aged 16+. Support and Accommodation provided enables the young people to lead a fulfilling life while Living Independently. Support is designed to help young people take control of their lives while being supported by our team when needed.We also support young people with learning disabilities, autism, brain injuries and complex needs.
